What to Expect When Enrolling for Small Group Coverage

Required Information & Documents

Be prepared to provide the following information when requesting a quote:

  • Company name
  • Desired insurance start date
  • Company ZIP Code (headquarters location) and county
  • Number of full-time eligible employees (including yourself)
  • Age and gender of each employee

To further tailor the monthly estimates and complete the enrollment application, you’ll be asked to provide the following:

  • Designated dependents for each employee
  • Selection of contribution strategy (percentage vs flat dollar amount)
  • Proof of wages documentation
  • Enrollment status for each employee (must provide a reason for declined coverage)

After uploading all required documentation and electronically signing your group coverage application, your application will be submitted for review, and you’ll be notified of any status changes. Does Your Company Qualify for Small Business Health Insurance?

You must employ at least two full-time employees, one of which may include the owner. For group coverage purposes, Underwriter approval is required for full-time employees that work 20-30 hours a week and for whom income tax is withheld. No approval is needed for those who work more than 30 hours a week.

In the state of Illinois, 70% of your full-time eligible employees must select coverage.

You must contribute a minimum of 25% to your employees’ premiums. Choosing to contribute more can make your plan more affordable, although it is not necessary.
