Vision Frequently Asked Questions

If you’re considering providing insurance coverage for your employees, we’re here to help. We’ve shared answers to some of the most common questions small business owners have about coverage below.

  1. Find an in-network eye doctor. Our Network Provider Search has thousands to choose from. Be sure to select your network. If logged in to Member Web, your correct network will display.
  2. Schedule an appointment; with many providers, you can do it online. Tip: It’s a good idea to call your doctor’s office to make sure they accept your benefits.
  3. When you arrive for your appointment, give the provider your name, date of birth and let them know you’re a member. They’ll be able to look you up instantly. You can also show your member ID card. If you need a copy of your card, you can download and print one when you log into Member Web.
  4. Pay for any copays and anything above your allowance. Your provider will tell you the amount.
  5. If you’ve visited an in-network provider, your eye doctor will file a claim and we’ll do the rest. If you’ve gone out of network, you’ll need to fill out an out- of network claim form and follow the instructions for submission. To access the out of network form, log in to your member account and navigate to the Claims tab.

When you visit one of our in-network eye doctors, you won’t have to; we take care of all the paperwork. If you do choose to go out of network and your plan has out of network benefits, you’ll need to pay during the visit and then submit a claim form for reimbursement. Remember to upload an itemized paid receipt with your name included. To access the out of network form or to check the status of a claim, log in to your member account and navigate to the Claims tab.

Progressive lenses are categorized in different tiers. Log in to your member account to check what your plan covers for each tier and to find a full catalog of our progressive lens and anti-reflective tier classifications.

If they’re under the age of 18, you can see their eligibility, benefits, claims and account information in Member Web. But, due to privacy rules, you will only see the name of enrolled dependents 18 or older in your account. You will not be able to see any eligibility or claims information. Dependents 18 or over will need to register for their own account.

Members get discounts on LASIK to help correct astigmatism, nearsightedness and farsightedness. Log in to your member account for specific details about LASIK savings and other discounts at participating in network providers.

As a vision benefit member, you have access to regularly updated discounts on vision- related products and services. These include everyday offers like 40% off a complete additional pair of glasses or 20% off the balance of frames, lenses or lens options when your benefits run out.
