Reduce Chronic Conditions with Metabolic Health Management

Poor metabolic health and many chronic conditions go hand in hand. Of the conditions associated with poor metabolic health, diabetes and obesity are among two of the biggest cost drivers facing American employers.
Metabolic health is measured by how well a body processes food and nutrients. When the body processes nutrients well, risks decrease for developing type 2 diabetes, heart disease, stroke, kidney disease and obesity. This cluster of conditions can include increased blood pressure, high blood sugar, excess body fat around the waist and abnormal cholesterol or triglyceride levels.
Diabetes, which can cause serious complications, is estimated to cost $413 billion in medical costs and lost productivity in 2022. People with diagnosed diabetes now account for one of every four health care dollars spent nationally.
Eligible members (those meeting clinical criteria) are invited to participate in the program, complete a health evaluation and meet with a clinician. Successful applicants are placed in the appropriate program based on their health needs.
Twin Health
Collaborating with our partner, Twin Health, members accepted into the program use continuous glucose monitors, activity and sleep trackers and surveys to capture diet, mood and exercise. A smartphone app translates this data into instant, personalized advice to help achieve health targets. Under clinician supervision, costly oral and injectable medications are slowly reduced or eliminated. Members showing sufficient progress continue as guided by the coaching team; those who don’t may be eligible to try another program in our Metabolic Health Program at any time.
Wondr
We also work with Wondr, a program specifically available as a weight management program. This skills-based, digital weight-loss program has helped people in all stages of health lose weight by using clinically proven health skills for weight reduction. Members learn from experts about how to make small, sustainable changes that can fit into their lifestyle.
Omada
Omada aims to reduce the risk of type 2 diabetes and heart disease by managing obesity-related chronic conditions through diabetes prevention and hypertension management programs. As a behavioral medicine program, it enables members at risk for these chronic conditions to change the habits that put them most at risk. The program combines data-powered human coaching, connected devices and a tailored curriculum to effectively improve overall health and reduce the risk of diabetes and cardiovascular disease.
Teladoc
Teladoc currently offers four member programs: a Prediabetes Management, Diabetes Management, Hypertension Management and a bundle called Chronic Condition Management Plus. CCMP provides personalized, holistic management of frequently comorbid conditions (obesity, dyslipidemia, mental health) based on diabetes, prediabetes or cardiovascular anchor conditions.
Members interact through the same Teladoc app or web portal and work with the same dedicated coach whether they participate in one or more stand-alone programs or Chronic Condition Management Plus. Members can access these supplemental digital condition management services at no cost to them and from the safety and comfort of their own homes.
