BCBSIL HMO Network Updates to The University of Chicago Medical Center and Affiliated Physicians Group, LLC

The following changes apply to these HMO provider networks:

• HMO Illinois^®
• Blue Advantage HMO^SM
• Blue Precision HMO^SM

Illinois employers will be notified later this week in their regular newsletter. BCBSIL will notify affected HMO group administrators, and group and retail members of the network change by mail. Please share these changes with your clients as appropriate. You can instruct members with questions to call Customer Service at the number on their member ID card.

• The University of Chicago Medical Center #491 will no longer be in the HMO Illinois provider network, effective Jan. 1, 2020.
  
  
• Affiliated Physicians Group LLC #448 will no longer be in the HMO Illinois, Blue Advantage HMO and Blue Precision HMO provider networks, effective Jan. 1, 2020.
  

Current members of The University of Chicago Medical Center (HMO Illinois) and Affiliated Physicians Group LLC (HMO Illinois, Blue Advantage HMO or Blue Precision HMO) will need to select a new HMO medical group.

Medical Group Selection Due Nov. 1
BCBSIL is requesting that members notify us with their medical group selection by Nov. 1, 2019. Member instructions will be included in the notification letter to assist members in this process.
